Nurse-led clinical support
Community Nursing
Clinical nursing care delivered in your own home, on your own terms — by registered nurses who know your history and notice when something changes.
Community nursing brings qualified clinical care out of the hospital and into the place you actually live. For NDIS participants it means wounds get dressed properly, medications get managed safely, and health problems get picked up early — without the exhausting logistics of getting to an appointment.
At Kalgi Care every nursing visit is planned and overseen by a Registered Nurse. We document what we find, we escalate when we need to, and we keep your GP, allied health team and family in the loop.
What community nursing covers
Wound and pressure injury care
Assessment, dressing selection, wound photography and healing reviews — including chronic wounds, surgical wounds, skin tears and pressure injuries.
Medication management
Webster pack checks, administration, insulin, eye drops, and monitoring for side effects or interactions. We also spot when a medication list has quietly stopped making sense.
Continence and catheter care
Indwelling and suprapubic catheter changes, bag management, urinalysis, and continence assessment and planning.
Health monitoring
Blood pressure, blood glucose, oxygen saturation, weight and skin integrity checks — tracked over time so trends are visible, not guessed at.
Post-hospital support
Discharge follow-up in the first vulnerable weeks home: wound reviews, medication reconciliation and escalation if recovery is not going to plan.
Care planning and training
Written nursing care plans, and training or delegation for support workers and family carers where that is safe and appropriate.
Who community nursing suits
- Participants with wounds, ulcers or pressure injuries that need regular clinical review
- People managing diabetes, continence needs or complex medication regimes
- Participants recently discharged from hospital or rehabilitation
- Anyone whose support workers need clinical guidance to deliver care safely
- Families who want a qualified clinician keeping an eye on things
How this is funded
Community nursing is usually funded from Improved Daily Living (Capacity Building) or from Core Supports — Assistance with Daily Life, depending on how your plan is written and what the nursing is for. If you are not sure which budget applies, we will look at your plan with you and explain it in plain language before anything is agreed.
